高中起點升專科、本科:應屆和往屆高中、中專、職高畢業生及同等學歷者。

專科升本科:已取得或將取得國民教育系列專科文憑者。

(一)擁護中國共產黨的領導,擁護四項基本原則,遵紀守法,品德良好,身體健康。

(二)在職、從業人員,高中、中專、技校應屆畢業生及社會其他人員。

(三)報考高中起點本、專科考生應具有高中畢業文化程度,報考專科起點本科及第二專業專科的考生必須是已取得經教育部審定核准的國民教育系列高等學校或高等教育自學考試機構頒發的大學專科畢業證書的人員。

(四)衛生部辦公廳、教育部辦公廳、國家中醫藥管理局辦公室釋出關於對成人高校醫學門類專業考生報名資格審查工作的補充通知。按照《教育部衛生部關於舉辦高等醫學教育的若干意見》的有關要求,該通知對成人高校醫學門類專業招生的考生報名資格稽核工作提出以下意見:一、考生報考成人高校醫學門類專業除符合《教育部關於做好全國成人高校招生工作的通知》附件中規定的報名條件外,還須符合下列要求:

1、報考臨床醫學、口腔醫學、預防醫學、中醫學專業的人員必須已取得省級衛生行政部門頒發的相應類別的'執業助理醫師及以上資格證書,或者縣級及以上衛生行政部門頒發的鄉村醫生執業證書並有中專學歷或中專水平證書。

2、報考護理學專業的人員原則上須取得省級衛生行政部門頒發的護士執業資格證書。

3、報考醫學門類其他專業的人員必須是在職衛生、醫藥行業技術人員。

4、考生所報考的專業原則上應與所從事的專業對口。

  成考考試確認報名所需材料

1、考生在網報後,前往區縣招辦確認,須按本人在網上選取的報名確認點和日期辦理報名資格審查、攝像手續,簽署《考生誠信考試承諾書》等。

到時需攜帶的有關材料:

網報號

身份證原件(25歲以上(1981年2月28日前出生)或少數民族考生需要影印件1張)。

高中畢業證書原件和B5影印件1張(19歲以下考生(1987年3月1日以後出生))。

大專畢業證書原件和影印件1張 (報考專升本的考生)。

照顧證明原件和B5影印件1張(符合各類照顧條件的考生)。

暫住證(外地考生)

2、前往的區縣確認點,必須是網報時選擇的區縣確認點。

3、網報期間請認真核對本人基本資訊和志願資訊,在報名點確認時將不能更改。

4、如果網上交費不成功,可以在報名確認時在區縣報名點補交費。對於網上多次交費的考生,考試院在報名結束後統一退到銀行卡中,考生可查驗銀行卡交易明細。

報名期間各區縣成招辦設報名確認點。報名確認手續必須由考生本人辦理。未辦理報名確認手續報名無效。

IQ(intelligence quotient智商)is a score that shows a person’s level of intelligence. People used to believe that some people are born with a high IQ and some aren’t.
